The inflammatory phase of healing is often where wound healing stalls or fails. Wounds that do not improve or that worsen after 6 weeks of standard care are considered chronic or hard-to-heal wounds. The TIMERS framework of wound bed preparation and the "step down, step up" approach are evidence-based protocols that promote wound healing. If chronicity develops despite these measures, wounds may respond to advanced therapies such as cellular and/or tissue-based products or negative pressure wound therapy.
